The symptoms of stage two stress breakdown: loss of emotional control and failure of self-motivation indicate an urgent need for rest and sleep.

The cause of the symptoms of stage two is the run-down of the body’s inhibitory reserve, just like an automobile battery which has run flat. The body’s reserves will therefore need to be regenerated. The treatment for second stage stress breakdown symptoms will require emphasis on relief of stress, rest, extra sleep and adequate nutrition. Sleep is the most important factor in the speed of regeneration of the body’s inhibitory reserve.

The person suffering from stage two stress breakdown can be thought of as a person experiencing a functional health crisis. The proper course of action in treating someone with second stage stress breakdown, should be:

1. The person needs to be rescued from the stressful situation by someone who can step in and arrange for the over-stressed person to be relieved of responsibilities. In most cases, the rescuer role is performed by the family doctor, who has the authority to issue sickness certificates, as well as to influence his patient to rest.

2. The patient should go to bed, hopefully to sleep. Meals should be regular, and problems and relatives should be kept away from the over-stressed person.

3. Drugs should be avoided for two reasons. Firstly, the patient has already demonstrated loss 0f emotional control, hence the diagnosis of second stage, people suffering from stage two stress breakdown might be more likely to take impulsive mini-overdoses, and secondly, the over-stimulation of the cerebral cortex, which results from the body’s reaction to sedative drugs, would be theoretically likely to provoke stage three stress breakdown symptoms.

Angry outbursts, or what some of my patients refer to as temper tantrums, can seriously disrupt the family. They’re a particularly difficult aspect of BDD. The outbursts usually reflect frustration and anger over the psychological pain, isolation, and disruption of life that BDD can cause.
Sometimes the outbursts are precipitated by a painful event, such as the perception of being mocked. Some of my patients routinely become very angry after receiving a “bad” haircut. Unfortunately, you may bear the brunt of this frustration and anger.
Try to understand that this behavior is related to BDD. In many cases, people with BDD don’t intend to express their anger toward you; you may just happen to be present when they’re feeling frustrated. Identifying precipitants of outbursts and trying to avoid them in the future may be useful. If getting into discussions or arguments about whether the defect is real—or looks really bad— triggers angry outbursts, then do everything possible to avoid these discussions. To accomplish this, try the approaches I recommended in the section on reassurance seeking. Setting limits on destructive behavior and urging the person to talk about how they feel, rather than expressing their feelings in angry behavior or attacking words, is also recommended. As BDD symptoms respond to treatment, the frustration and anger usually diminish as well.
It may be helpful to all meet together with the treating psychiatrist or therapist to discuss the outbursts and their effect on the family. Family discussions or family therapy may yield helpful strategies for dealing with anger that are tailored to your family and may make this problem easier to cope with.

If BDD upsets your family routine, try to get it back to normal. This may take some time. If the symptoms don’t upset your routine, try to prevent this from happening. No one benefits when BDD takes over and disrupts your life. It’s best for both you and the BDD sufferer to live as normal a life as possible. For example, if you’re planning on having relatives over for Thanksgiving even though your daughter is going through a bad spell and doesn’t want to see anyone, go ahead with the dinner and encourage her to participate. Do this without expressing anger or criticism, and don’t feel guilty about it. It’s best for everyone to keep your life on track so you don’t feed the BDD. You’ll also feel less resentful, have more energy to help the BDD sufferer, and enjoy your own life more. Consider getting professional help for yourself if you need more support and help with coping.

If alcoholism is so highly treatable, what has been going wrong? Why aren’t more people receiving help? The obstacles do require examination. Historically, one big handicap has been society’s attitude toward alcohol and its use. Unfortunately, the alcoholic’s chances of being treated for alcoholism have long been slim. Oh, he will receive treatment: for depression, gastritis, cirrhosis—but less often for the alcoholism. Despite all the public education and information, the notion remains lurking that talking about someone’s drinking is in bad taste. It seems too private, somehow, none of anyone’s business. Most of us have a good feel for the taboo topics— sexual behavior, people’s way of handling their children. The way someone drinks has been a strong taboo. Of course, an alcoholic has a stake in keeping the drinking and its associated problems off limits. Should the drinking behavior be discussed, the alcoholic’s rationalization combined with the tendency to “psychologize,” analyze, and get to the reason would spring another trap. The notion that someone drinks alcoholically only because he is an alcoholic sounds circular and simpleminded. Thus, everyone scurries around to find the “real” cause. Alcoholism as a phenomenon, a fact of life, gets pushed aside and forgotten in the uproar.
Another obstacle is the confusion introduced for alcoholics and their families by the behavioral symptoms of alcoholism. One common characteristic of alcoholic behavior is the extreme variation, the lack of consistency, sometimes good mood, sometimes foul mood, sometimes sloshed, sometimes sober. This inconsistency invites a host of explanations. Further, this very inconsistency allows the alcoholic, family, and friends to hope things will get better if left alone. It permits them to delay seeking assistance. It almost seems to be human nature to want— and wait—for things to improve on their own. Consider for a moment the simple toothache. If the toothache comes and goes, you probably will delay a trip to the dentist. After all, maybe it was something hot you ate? Maybe it was caused by something cold? Maybe, or maybe… ? On the other hand, if the pain is constant, if it is clearly getting worse, if you can remember wicked toothaches in the past, you’ll probably call immediately for an emergency appointment with your dentist. The total time you are actually in pain in the latter case may be much less than what you would have put up with in the former example, but you are spurred into action because it doesn’t look like it will improve by itself.
Unfortunately, even when alcoholism treatment is instituted, it can be seriously unbalanced, zeroing in on only a part of the symptomatology. When this happens, alcoholism treatment can end up resembling treatment for depression, or cirrhosis, or just a “rest cure.” These one-sided approaches are major sources of recidivism and failure, and not infrequently they have given the impression that alcoholism treatment doesn’t work.

The Metamorphic Technique is another highly relaxing treatment which can have you nodding off during a session. In addition, people who have regular sessions find themselves changing for the better, becoming better able to cope with problems and clear about what they want in life. The tensions and anxieties leading to sleeplessness can drop away in the process.

The technique is a very gentle form of massage of the feet, hands and head — principally the feet. It is believed to stimulate the receiver’s powers of self-healing by releasing emotional blocks. It was developed by an English naturopath, Robert St John, after studying reflexology. Reflexology works on the whole foot and on physical symptoms; in the reflexology system the inner side of the foot, from the big toe to the heel, represents the head and spine. St John discovered intuitively that this area also represents the period of conception, gestation and birth, and that gently massaging it can release emotional traumas experienced during this vital time.

Children can benefit enormously from the Metamorphic Technique, and often enjoy giving sessions to other people. A good deal of work has been done on children with Down’s Syndrome and other problems; the technique can also soothe children who are hyperactive or highly strung. Adult stroke patients, and people with nervous disorders have also benefited from it. The technique is not a form of medicine and its practitioners do not claim cures; it seems to have the effect of allowing the person to reach his or her full potential, whatever that potential happens to be.

Self-help and further information

The Metamorphic Technique is easy to learn and practise, with your friends, partner or children; sessions should not be given more than once a week. It’s also possible to treat yourself, though this is never as relaxing as receiving a treatment from someone else.

Instructions are given in The Metamorphic Technique by Gaston St Pierre and Debbie Shapiro (Element Books, 1982). Personal tuition is advisable to start with, however; as well as the very simple technique, practitioners need to learn to adopt a detached attitude, unconcerned with getting results — something not everyone finds easy.

Herbal medicine has been used by mankind throughout the ages and all over the world, and is growing in popularity today in response to concern about drugs, and the desire for more natural forms of medication. Herbalists, like any other natural practitioners, feel that relying on herbs simply as tranquillizers is much the same as relying on medical drugs unless you also deal with the causes of your insomnia. The herbalist’s aim is not merely to treat symptoms, but to prescribe medicines that will improve general vitality clear the system of toxins, and restore balance and harmony; good sleep then comes about naturally.

Many modern drugs are based on plants: aspirin is extracted from willow, and digitalis from the foxglove, for instance. What modern science has done is to isolate from these plants, the ‘active ingredient’ that provides relief or cure. What it has overlooked in this process is that each herb contains a balance of ingredients which counteract any side effects from the active ingredient taken in isolation. They also contain health-promoting vitamins and trace elements. Herbalism is therefore generally very safe and properly prescribed medicines have no side-effects. It is true, of course, that some herbs are harmful, and you should go to someone who has been properly trained.

Herbalists treat much the same range of problems as GPs, and their diagnostic techniques resemble those of medical doctors, using the same equipment for testing blood pressure and so on. As well as assessing symptoms, practitioners evaluate the overall balance of the body’s various systems to ascertain underlying disharmonies, and they prescribe on the basis of the whole person rather than symptoms alone. Therefore, as with homoeopathy and other natural medicines, prescriptions for the same disease will vary for individual patients. Different herbs would be appropriate, for instance, for sleeplessness caused by anxiety, digestive problems, hormonal imbalance, and so on.

For insomnia, the herbalist would want to find out what is contributing to lack of sleep. Practitioners look into the patient’s lifestyle, including exercise and nutrition, and will recommend a healthy, wholefood diet; treatment is regarded as a co-operative effort in which patients play their part by making any changes that are indicated.

Medicines are usually dispensed in liquid form as tinctures. Herbal medicine can act quite fast, particularly when patients pay attention to a good diet. In chronic cases, however, it can take time to restore health as the medicines work gently and thoroughly, both detoxifying the patient’s system and building up his or her strength.

Self-help

Herbal medicines can be useful to try out at home for minor health problems in adults and children, and you can learn to use them from a number of books including those listed below. If you take a herbal remedy for a chronic condition, you can expect some improvement within two or three days, but it may take two or more weeks to get the full effect. So take any remedy for a month to give it a fair trial, and when you do improve, taper off gradually. Herbs are not truly addictive, but since they act upon the central nervous system they should not be taken regularly for weeks on end.

I have often been told people can’t let an attack or the anxiety happen because other people may see it happening. So what if they do. Why are we giving our mental health away to everyone else? We can spend all day trying to hide our symptoms from employers, work colleagues, family and friends. The extraordinary energy and control we use to hide our symptoms only makes us more anxious and exhausted. The more anxious we become the more we have to hide it.

Taking the power back means we cannot let the fear of what other people think get in our way of full recovery. If our hands and legs shake, let them shake. If our face turns bright red, then our face turns bright red. If we feel faint, then sit down on a chair, on the floor, on the footpath, if need be. If we vomit or have an attack of diarrhoea, then we vomit or have an attack of diarrhoea. Let it happen. When we let it happen, we turn off the adrenalin and it will be over as quick as it starts. We will not have to waste all of our energy trying to keep it under control and thereby turning on more adrenalin. Our mental health needs to be more important than other peoples’ opinions. The feelings of embarrassment are created by our thoughts. We have to move from ‘what if to the all powerful attitude of ‘so what’. ‘So what’ if we have an attack, ‘so what’ if we are feeling anxious, ‘so what’ if people see. So what.

Depending on how high our anxiety level is, the anxiety may not disappear as quickly. Learning to manage the anxiety by being aware of our thoughts, letting them go and by letting the anxiety be there, is part of the recovery process. As we work through the process of recovery our anxiety level diminishes, until we are anxiety-free.

We will reach the point where we will have a choice in how we respond, either with fear or by letting it happen. This choice will always be there. After recovery in times of extreme stress we may experience further attacks. We can choose how we respond: either with fear, ‘what if, or by letting go and letting it happen. So what.

The working through process can at times be very frustrating, but the final result is worth every step. Everything which has been taken away from us by the disorder will be given back to us through the clarity of thought and freedom wbich recovery brings.

All people—children and adults—move through distinctly different sleep states which progress from drowsiness to very deep sleep. In the waking state we are rational and functional. In non-REM sleep the body rests and restores itself. In REM sleep the mind is active again and dreaming occurs (REM stands for “rapid eye movement”—a stage of sleep that is characterized by such eye movement.)

The night begins as the child moves quickly through drowsiness and into deep sleep within ten minutes. Waking a child in deep sleep might be almost impossible—this is the time your child can be moved from the car or your arms into bed without waking.

Deep sleep makes up the next two or so hours of sleep. After about an hour (and again after the next hour), it is interrupted by a brief arousal when the child seems to be sleeping and waking at the same time. Behaviors range from crying out or opening eyes to the more extreme thrashing about. It is during this arousal that confused thrashing, sleep terrors, sleep walking, or bedwetting might occur. Although we tend to think that these behaviors are a result î dreaming, that is not the case since dreaming does not occur until REM sleep

The bulk of the remaining night is spent moving between light sleep and the REM episodes in which dreaming occurs. There is a tendency to wake briefly while changing states—the child checks to be sure all is well, goes back to sleep, and generally doesn’t remember this waking in the morning. Sometime when he wakes during these “arousals” he has difficulty returning to sleep. This is a common cause of frequent waking. If he needs your help to get back u sleep, he will wake you.

It is during this block of time (in the REM episodes) that nightmares occur. If he comes to a full waking while moving in and out of the dreaming states, he might be afraid to fall asleep again.

Near morning he returns to another period of deep, non-REM sleep. Following another arousal comes another, more intense, REM dream episode. After more light sleep and dreaming transitions, he wakes for the day. If the child wakes fully during any of these transitions, he might decide that night is over; he then becomes an early waker. If you decide to wake him (to fit your schedule or in an attempt to alter his) during his period of deep sleep, you will both be left out of sorts.

A number of factors may combine to cause bronchial asthma. Of these, allergy, infection, genetic constitution, and emotional influences are the most important. However, as is often the case in the history of medicine, the last twenty or thirty years have seen an overemphasis on one of these factors at the expense of the others. There has been a remarkable preoccupation with the allergic factors of asthma. The preparation of antigens and their Use in skin testing and desensitization has all the appeal of being scientific. However, this preoccupation with allergy has led to the neglect of the emotional factors, which are much more elusive and harder to appreciate as an aspect of science. But the importance of the emotional influence is beyond all doubt.

I have bad a number of patients who suffered for years from classical asthma with proven sensitivity to common pollens and dusts. They ceased to have attacks of asthma after being treated by relaxing methods, even though they were still exposed to the same pollens and dusts which in the past had caused the attacks. Furthermore these patients have shown no formation of substitute symptoms, such as skin rashes, that have been reported by some authorities when hypnosis was used to stop asthma by direct suggestion. I could quote histories of a great number of patients whose asthma was either completely relieved or at least greatly improved following the practice of relaxing mental exercises.

Perhaps one of the most remarkable was a woman of forty-six who had suffered from severe asthma for thirty years. She seemed to live by repeated use of her spray. After practising the relaxing exercises, the woman simply ceased to have attacks, and her spray was completely abandoned. I am sure that if there had been any recurrence she would have returned to me, but she has not done so.

A lad of eighteen was incapacitated by asthma. He ceased to have attacks, and I lost contact with him until a relative came to consultation a few weeks ago, and reported that the lad had remained free of asthma since seeing me five years ago.

A young married woman was extremely sensitive to house dust, and any housework always produced a severe attack. After some relaxing treatment she came to be able to dust the house in the normal way without any ill effect.

The significant finding of my work in this field is that many patients who suffer from classical bronchial asthma cease to have attacks when the general level of their anxiety is reduced. Some other patients have continued to have attacks, but they have been much less frequent and much less severe. Other patients have remained uninfluenced by this approach. Approximately one third of the patients so far treated fall into each of these three categories. It would therefore seem that anyone suffering from bronchial asthma should at least try the method of relief through the principles of self-management. The method reduces the general level of anxiety in a way similar to the relaxing method I have used in my office treatment.
